Live Well Mercer County is a partnership between the Mercer County Health District, Foundations Behaviorl Health Services, Mercer County Educational Service Center, Tri-County ADAMHS Board (Mercer, Paulding and Van Wert Counties), Our Home Family Resource Center, Mercer Health, and others.
With a mission to enable all individuals to Live Well in Mercer County, one of the important initiatives of Live Well Mercer County is community health needs assessment (CHNA). Every three years this collaborative group conducts a CHNA to determine the health needs of Mercer County, then develops a community health improvement plan to help address those needs. The cornerstone of the CHNA is a community survey. It is important that as many Mercer County residents as possible complete the 2024 Community Health Needs Assessment Survey so action plans can be created to address needs in Mercer County.
